Be vigilant: lymphoma also grows on the bones

BY Iris Wheatley 2020-08-01

Recently, some friends suddenly asked me about the medical knowledge of lymphoma. After further understanding, I realized that everyone is very concerned about this disease. In fact, lymphoma is mainly a tumor of the blood system. As an orthopedic surgeon, I learned that lymphoma can also infect human bones.

In clinical work, sometimes Some patients with bone tumors come to see you because of long-term pain, or because they are found late, and then see a pathological fracture before seeing a doctor (pathological fracture: caused by damage to the whole body or local parts of the bone itself) fracture). Among them, the early detection of bone lymphoma has a better treatment effect.

Bone lymphoma is not common compared to other bone tumors. The main cause of the disease is human immune dysfunction. Some cases are related to human infection with Epstein-Barr virus. Other cases are related to long-term exposure to chemicals (such as: Insecticides). The disease usually occurs in men around 40 years old, and rarely occurs before the age of 20 years. In the early clinical period, the systemic symptoms of bone lymphoma are often similar to colds, which can cause repeated fever, uninduced weight loss, liver and spleen, and lymphadenopathy. Most of the local symptoms are pain, often persistent dull pain or severe pain. Unlike the pain caused by general strain, the pain of bone lymphoma is often worse at night, and sometimes it is also shown as radioactive pain.

Bone lymphomas occur for three reasons: ①The adjacent diseased lymph nodes directly invade the bones, and the lesions are more common in the spine, pelvis, ribs and sternum; ②The tumor cells in the body are transferred by the flow of blood or lymph fluid. 3. Perfusion into the bone; ③ Bone marrow tissue occurs under the influence of pathogenic factors. This type is also called primary lymphoma of bone, and the possibility of cure is great.

Through X-ray examination, the image of the primary lymphoma of the bone is as follows: the bone is "eaten" little by little by the insect (medically called: worm-like), the border is blurred . Isotope bone scan and magnetic resonance examination can further confirm the diagnosis.

In terms of treatment, primary lymphoma of bone is sensitive to radiotherapy, and combined with chemotherapy, multiple courses of treatment are required.
